Pour the milk, heavy cream, and sweetened condensed milk into the crockpot.
Whisk in the cocoa powder, then add the chopped semi-sweet chocolate on top.
Cover and cook on low for 2 hours, stirring every 30 minutes to help the chocolate melt evenly.
Once the mixture is fully melted and smooth, stir in the vanilla extract.
If the hot chocolate is too thick, whisk in an extra splash of milk until it reaches your preferred texture.
Switch the crockpot to warm and ladle into mugs. Add marshmallows, whipped cream, or your favorite toppings.
